Community Wellbeing Specialist

This is an exciting opportunity for a Community Wellbeing Specialist to join our team in Cumbria, Northumberland, Tyne and Wear.
We are seeking a motivated and experienced professional to work within the Working Age Adults pathway, developing Recovery and Wellbeing focused Care Plans, and delivering high-quality individualised care.
Responsibilities- In collaboration with the Service User and Carer, develop and implement Care Plans that promote recovery and wellbeing.
- The post holder will work closely with the Multi-disciplinary Team to deliver exceptional patient care, maximising independence and promoting recovery.
To be successful in this role, you must have:
- Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
- Experience of working within a community team.
- Non-Medical Prescribing.
An estimated salary of £38,890 - £44,883 per annum, dependent on experience, reflects the importance we place on this role in delivering exceptional patient care.
