Job Title: Field Care Supervisor

We're seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of our care services in the community.

Key Responsibilities:

  • Support the planning and delivery of care services, working closely with care coordinators and care workers.
  • Conduct regular reviews of individual services, including risk assessments and needs assessments.
  • Provide supervision and support to care workers, ensuring they have the necessary skills and knowledge to deliver high-quality care.
  • Participate in multi-disciplinary reviews of service users' needs, working with healthcare professionals and other stakeholders.
  • Shadow new care workers to ensure their competence and provide on-site observation and evaluation of care worker practice.

Requirements:

  • Experience in carrying out initial risk and needs assessments, working with service users, their families, and healthcare professionals.
  • Ability to work in partnership with service users, their families, and carers to deliver holistic packages of care support.
  • Strong communication and interpersonal skills, with the ability to work effectively with a diverse range of stakeholders.
  • Ability to work independently, with minimal supervision, and as part of a team.
