The post holders will support in facilitating effective, efficient, and timely discharge from hospital for our pathway 1-3 patients, ensuring a home-first approach. This is an ideal opportunity to be involved in the transformation of our Integrated Discharge processes and enhance partnership working with nurses, therapists, and external agencies.

The main duties of the role include actively assisting and supporting patients and staff in discharge planning and processes, ensuring a smooth hospital flow. If you have good communication skills and can engage with multiple professionals, patients, and their families to ensure discharge home is at the centre of planning, this is the opportunity for you.
