Job Summary:
This position involves working with adults aged 18+ who require support due to various reasons such as frailty, illness, or traumatic events. The successful candidate will be part of a dynamic team based in hospitals, interacting with patients, colleagues, and community-based settings like reablement units, care homes, and individuals' residences.
Key Responsibilities:
- Conduct thorough assessments and provide personalized support to vulnerable adults.
- Collaborate with healthcare professionals, families, and caregivers to ensure comprehensive care plans.
- Develop and implement effective strategies to promote independence and improve quality of life.
Requirements:
- Degree in Social Work or related field.
- Registration with the Care Council for Wales.
- Excellent communication and interpersonal skills.
