About the Role
Rethink Mental Illness is seeking a dedicated Care Navigator to provide tailored support to individuals being released from prison. As part of our integrated mental health team, you'll work closely with healthcare and prison staff to ensure a smooth transition from prison to community life.
Key Responsibilities
- Deliver pre- and post-release support to individuals with health vulnerabilities, connecting them to community services.
- Conduct initial assessments and care mapping for service users, creating tailored care plans for effective release and reintegration.
- Organise and contribute to health promotion events, supporting the reduction of health inequalities and the promotion of wellbeing.
- Maintain accurate, detailed records in accordance with organisational and national standards.
Qualifications & Skills
- NVQ Level 3 or equivalent in Health and Social Care or related field.
- Experience supporting vulnerable individuals to set and achieve goals, with knowledge of health, criminal justice, housing, and social care.
- Strong communication skills with experience in case management, structured interventions, and care planning.
