We are seeking an experienced and passionate Social Worker to join our dedicated Adult Long Term Planning Team based in the South East. As a key member of our team, you will be responsible for working with adults who require support and guidance to maintain their independence and well-being.

Key Responsibilities:
  • Carrying out Care Act assessments and safeguarding work
  • Working in accordance with social care legislation and local government requirements
  • Establishing and maintaining relationships with service users and their families

Requirements:
  • Previous experience in Adult Social Care
  • Good understanding and application of relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005
