About the Role

The successful candidate will be responsible for providing high-quality physical health support to individuals with complex mental health conditions. This will involve conducting thorough assessments, developing care plans, and coordinating with healthcare professionals to ensure seamless care delivery.

You will work closely with our multidisciplinary team to identify patient needs, prioritize care, and implement effective interventions. Your expertise in physical health support will enable patients to manage their conditions more effectively, improving their overall quality of life.

In exchange for your dedication and expertise, we offer a competitive salary range of £28,000 - £38,000 per annum, plus generous benefits and opportunities for professional growth and development within our organisation.
