A Care Worker - Assisted Discharge will be responsible for taking patients home from hospital, ensuring they have basic resettlement needs met, and building positive relationships with service users, their families, and other healthcare professionals.

  • Previous experience in a similar care setting, preferably within a healthcare or social care environment.
  • Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals' lives.
  • Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team.
  • Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
  • Flexibility to work various shifts, including evenings, weekends, and bank holidays.
