About This Role

We are seeking a highly skilled Adult Care Specialist to join our team at Queens Park Medical Centre. As a member of our multidisciplinary team, you will be responsible for delivering high-quality patient care and support.

Key Responsibilities:
  1. Patient Care Coordination - Develop and review care plans with patients, families, and carers. Identify patients needing coordination and ensure they receive appropriate support. Manage appointments, address queries, and provide information to help patients make informed choices. Collaborate with healthcare professionals to ensure cohesive care.
  2. Multidisciplinary Team Support - Participate in MDT meetings to coordinate care for vulnerable adults and those at risk of emergency hospital admissions. Serve as a central contact, ensuring care plans are communicated to all relevant professionals.
  3. Administrative Support - Perform general administrative tasks as assigned by the PCN Manager. Record meeting minutes, manage patient records, and organise meetings.
  4. Patient Support and Advocacy - Support patients in understanding and managing their conditions. Help patients access self-management education, peer support, and personal health budgets where applicable.
  5. Administrative and Professional Development - Maintain up-to-date care records using health platforms. Engage in ongoing professional development, including role-specific training.
  6. Key Skills and Competencies - Strong written and verbal communication skills. Excellent organisational and time-management abilities. Empathetic, patient-centred approach to care.
  7. Confidentiality - Uphold patient confidentiality and ensure all information is managed in line with policies and data protection regulations.
  8. Equality, Diversity, and Inclusion - Promote the rights of patients, carers, and colleagues with a respectful, non-judgemental approach.
  9. Health and Safety - Follow health and safety policies, report incidents or concerns, and maintain a safe working environment.

Salary: £38,000-£48,000 per annum.
