Community Nurse Lead

We are seeking a highly motivated and organized Community Nurse to lead our Community Nursing Team in Tiverton and the surrounding area.
The successful candidate will be responsible for ensuring high-quality, evidence-based care for our patients. They will work closely with colleagues across the Community Health and Social Care Pillar, including Urgent Community Response, Community Rehab, and Adult Social Care.
As a key member of our team, the Community Nurse Lead will be responsible for managing a team of community nurses, ensuring effective day-to-day operational leadership, and delivering the Community Nursing service within allocated resources.
The post-holder will work closely with the Clinical Matron and Community Services Manager to ensure that the cluster and practice population receive the appropriate care and treatment as close to home as possible.
