Job Summary
We are seeking a highly skilled Dietitian to join our team at Livewell Southwest. As a key member of our multidisciplinary team, you will play a critical role in delivering high-quality nutrition and dietetic services to our patients.
About the Role
The successful candidate will be responsible for providing specialist dietetic assessments, diagnoses, treatments, and education to patients with complex nutritional needs. You will work closely with healthcare professionals to ensure seamless, integrated care and provide advice, signposting, guidance, and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts.
Key Responsibilities
- Provide specialist, high-quality nutrition and dietetic services within Frailty Virtual Wards.
- Deliver specialist dietetic assessments, diagnoses, treatments, and education to individual patients.
- Provide advice, signposting, guidance, and information to health and social care professionals, patients, carers, relatives, or other non-professional contacts.
- Be an integral member of the multidisciplinary team, providing education, training, and dissemination of information to all healthcare professionals.
- Work as an autonomous practitioner, holding a clinical caseload and working without direct supervision.
- Continuously develop own professional skills and knowledge in line with HCPC regulations.
