About the Role

We are seeking an experienced and skilled Senior Community Nurse to join our team at Bridgewater Community Healthcare NHS Foundation Trust. As a Senior Community Nurse, you will be responsible for providing high-quality clinical leadership and management to our district nursing service in Halton Borough.

Key Responsibilities

  • Provide clinical leadership and management to the district nursing team, ensuring the delivery of high-quality patient care and services.
  • Develop and implement effective plans to improve patient outcomes and service delivery, working closely with the team leader and other stakeholders.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Conduct complex assessments and develop holistic care plans, taking into account the physical, emotional, and social needs of patients.
  • Provide education and training to junior staff members, promoting their development and growth within the team.

Requirements

  • Registered Nurse with a postgraduate qualification in a specialist field or working towards.
  • Experience in clinical leadership and management, with a proven track record of improving patient outcomes and service delivery.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to prioritize tasks, manage workload, and delegate responsibilities to team members.
  • Knowledge of clinical examination, clinical skills, tissue viability, palliative care, diabetes management, and frailty.
