Job Summary

We are seeking a committed and motivated Memory Assessment Nurse to join our team in the East/North East locality. The successful candidate will work part-time, 30 hours a week, with flexible working days from Monday to Friday.

Main Duties

The Memory Assessment Nurse will be responsible for assessing service users for signs of organic progressive disease, discussing findings with the multidisciplinary team, and developing a supportive care plan. Post-diagnostic support will also be provided, including practical information, signposting, and emotional support.

Job Responsibilities

The successful applicant will work independently but under the supervision of senior team members, receiving clinical supervision and a robust personal development plan. Experience and knowledge of memory assessments and evidence-based tools are desirable, as well as an in-depth understanding of the needs of service users with dementia.
