This position requires a Social Work Qualification with a minimum of 2 years post-qualified experience.
Key Responsibilities- Assess patients to ensure their needs are met and they can continue to live independently.
- Conduct assessments from home and on hospital wards as necessary.
- Manage time effectively to prioritize workload and engage with service users from diverse backgrounds.
- Degree in Social Work (Degree/DipSW/CQSW) with a minimum of 2 years experience.
- Excellent interpersonal skills and ability to work in a fast-paced environment.
- £34.00 per hour umbrella (PAYE payment options available).
- Hybrid working schedule.
- Parking available and easily accessible via car or public transport.
