Job Title: Rehabilitation Case ManagerAbout the Role

Active Care Group is seeking an experienced Rehabilitation Case Manager to join our team. As a key member of our care management team, you will be responsible for providing expert case management services to clients with ongoing needs.

Key Responsibilities:

  • Advocating for clients' needs and building strong relationships with families, deputies, and litigation solicitors
  • Developing and implementing individual client-focused support and rehabilitation plans
  • Recruiting, selecting, and managing a team of care/support workers
  • Monitoring and reviewing case management plans and support/rehabilitation plans
  • Providing a flexible working pattern to ensure a smooth operation of service
  • Compiling risk assessments and identifying risk management strategies

Requirements:

  • UK professional health qualification (HCPC, NMC, or Social Worker Registered)
  • Full driving licence and access to a car
  • Previous experience as a case manager (adults or children preferred)
  • Knowledge of CQC fundamental standards and key lines of enquiry
  • Working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), and Safeguarding frameworks
