About the Role

We are seeking an experienced Intensive Support Practitioner to join our team at Rivendell. As a key member of the team, you will work closely with children and young people with complex needs, providing tailored support in their daily lives.

Your Key Responsibilities:

  • To provide support to children and young people in accordance with their individualised plans.
  • To maintain accurate records of support provided, utilizing effective communication skills to record and report as necessary.
  • To contribute to a collaborative team environment, sharing knowledge and expertise to ensure high-quality care.

Our Requirements:

  • SVQ Level 3 in Health & Social Care (Children & Young People) and HNC.
  • Prior experience working with children and families in similar settings.
  • Strong communication skills for effective recording and reporting.
  • Driver's license with access to a vehicle.
