Derbyshire Early Intervention in Psychosis Service

We are seeking a skilled and compassionate Mental Health Case Manager to join our multi-disciplinary team.

The successful candidate will work with individuals experiencing a first episode of psychosis, providing holistic support and guidance to help them achieve their goals.

You will be responsible for assessing individual needs, developing care plans, and working collaboratively with families and carers to ensure the best possible outcomes.

About the Role

This full-time position offers a competitive salary of £38,890 - £44,884 per annum, plus benefits.

Key Responsibilities

  • Assess individual needs and develop personalized care plans
  • Work collaboratively with families and carers to ensure effective care coordination
  • Provide emotional support and guidance to individuals experiencing a first episode of psychosis
  • Collaborate with the multi-disciplinary team to achieve optimal patient outcomes
  • Participate in regular supervision and CPD activities to maintain professional competence

Requirements

  • NMC registration as a mental health nurse
  • Experience working with individuals with psychosis
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
