Job Summary

We are seeking a skilled and compassionate Registered Mental Health Practitioner to join our First Episode Psychosis Service and developing At Risk Mental State service.

Main Responsibilities
  • Collaborate with service users, carers, and system partners to deliver high-quality care.
  • Manage and oversee the care of service users, ensuring accurate and timely documentation.
  • Conduct comprehensive assessments, formulating personalized care plans that reflect the needs of service users and their support networks.
  • Attend multidisciplinary team meetings and report incidents as required.
  • Act as a Care Co-ordinator, providing education and support to develop service users' understanding of mental illness.
  • Provide psycho-social interventions, education, and support to prevent relapse and admission.
