Medicines Optimisation Expert

Responsibilities include:
- Providing pharmaceutical consultations and advice to patients, carers, and healthcare professionals.
- Contributing to the development and implementation of medicines optimisation strategies across the network.
- Managing therapeutic drug monitoring systems and overseeing the safe transfer of care between settings.
- Reporting medicines-related incidents and contributing to investigations and quality improvement initiatives.
- Participating in quality assurance and safety processes across the organisation.
Required Skills and Qualifications include:
- A degree in pharmacy and registration with the General Pharmaceutical Council.
- Strong knowledge of pharmacology and therapeutics, particularly in the context of primary care.
- Experience in medicines management, including prescription review and adherence support.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- A full driving license, as regular travel to practices within the network is required.
Salary and Benefits package includes:
- A competitive salary of £65,000 - £80,000 per annum, dependent on experience.
- A comprehensive company pension scheme.
- Sick pay and annual leave entitlement.
Monday to Friday Working Hours with some flexibility required to accommodate the needs of the Primary Care Network.
