Job Summary:

We are seeking a highly skilled and experienced Senior Adult Nurse Case Manager to join our team at Leaders In Care Recruitment Ltd. As a key member of our community care team, you will be responsible for providing face-to-face support to adult clients and their families, while creating personalized care plans that meet their unique needs.

Key Responsibilities:

  • Manage a reduced caseload of service users, providing high-quality care and support to ensure optimal outcomes.
  • Support the business growth plans by mentoring Adult Nurse Case Managers, sharing your expertise and knowledge to enhance the team's performance.
  • Design and deliver tailored care plans that address the specific needs of each client, taking into account their physical, emotional, and social requirements.
  • Effectively manage the multidisciplinary team (MDT) through strong communication and teamwork, ensuring seamless collaboration and coordination of care.
  • Provide ongoing monitoring, support, and assessment of clients' physical and physiological needs, identifying areas for improvement and implementing evidence-based interventions.
  • Deliver nursing interventions, including the management of medication delivery, to ensure safe and effective care.
  • Build and maintain sophisticated relationships with clients' families, providing emotional support and guidance to help them navigate the care system.
  • Work closely with community teams, CHC nurses, and CCG commissioners to establish and maintain bespoke packages of care that meet the unique needs of each client.

Requirements:

  • RGN with an active NMC PIN and up-to-date experience of tracheostomy care and ventilators within the last 2 years.
  • Supportive management style that can bring out the best in others, with a proven track record of effective leadership and management of HCAs.
  • Advanced communication skills, both verbally and in writing, to liaise effectively with patients, families, and the MDT.
  • Suitable for Band 5 / Band 6 Nurses with evidence of effective leadership and management of HCAs.
  • Full UK Driving Licence desirable but not essential.
