Key Responsibilities:

• To work collaboratively with multidisciplinary teams to achieve seamless care transitions and optimal patient outcomes.
• To assess, plan, and deliver holistic care to patients within their own homes and residential settings.
• To participate in Continuing Professional Development (CPD) opportunities to enhance clinical and interpersonal skills.

Requirements:
- Registered Nurse with current NMC registration
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Commitment to lifelong learning and professional development

Salary and Benefits:
The salary for this role is estimated to be around £25,000-£30,000 per annum, depending on experience. The successful candidate will also receive a comprehensive benefits package, including flexible working arrangements, occupational health support, and access to extensive training and development opportunities.
