Clinical Support Worker

Job Overview
The successful applicant will be responsible for working alongside the SWMS clinicians and supporting patients at specific community venues across the areas we cover. They will also be required to provide cover at other venues, including evening shifts at The James Cook Hospital or Friarage Hospital, Northallerton.
A day in the life of a Clinical Support Worker will include quick weight checks, where they will be responsible for taking a weight of a service user and discussing this, along with any questions the service user may have. They will also run longer review appointments, covering discussion of bariatric surgery and discussions about implementing advice given by the clinical team into the patients lifestyle.
The successful applicant will have previous experience of working in the NHS, and possess good written and verbal communication skills, as well as excellent team working skills.
Main Duties of the Job
Key Tasks
- Work with the SWMS Multidisciplinary team and Administration team to deliver an effective service for our clients.
- Help people in the Hartlepool, Teesside, Redcar & Cleveland, North Yorkshire, Sunderland, Durham and Darlington, and South Tyneside areas who are referred to the SWMS to lose weight and maintain weight loss.
- Interact with clients on both an individual and group situation.
- Feedback relevant information to the SMWS Multidisciplinary team and Administration team as appropriate.
- Support the SWMS Administration team with regular duties and cover to ensure an effective service for our clients and clinicians.
- Work closely with Tier 2 and Tier 4 weight management services.
- Contact other stakeholders as required to gather or request more information from GP surgeries.
- Promote the equality and value the diversity of individuals.
- Build up and maintain knowledge of contacts within the community.
Enable Individuals to Change Their Behaviour to Improve Their Health and Weight
- Help individuals identify how their behaviour and context might affect their health and weight.
- Help individuals develop personal goals and assist them to carry out advice from the SWMS Multidisciplinary team.
- Support individuals in maintaining their behaviour change.
- Help individuals to access and use local services as appropriate.

South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust. This training programme has been designed to support our leaders in developing their understanding of leadership and management skills. You will be expected to attend the New and Aspiring leaders programme, and the Foundation Quality Improvement Programme that the Trust delivers. After completing this one and a half day of training, you will be able to explore further leadership and improvement training opportunities, including in-house courses and leadership apprenticeships at level 3, 5 and 7.
