Mental Health Specialist Community Nurse

We are seeking a skilled and compassionate Mental Health Specialist Community Nurse to join our client's Adult Community Mental Health Team (CMHT) in Wiltshire. This is an exciting opportunity for an experienced professional to make a meaningful difference in the lives of patients.
Job SummaryThis role involves delivering high-quality community mental health services as part of a multi-disciplinary team. As a Specialist Community Nurse, you will play a crucial role in assessing patients' mental health needs, developing care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes.
About the Role- You will work closely with patients, families, and carers to provide comprehensive support and guidance.
- Participate in multi-disciplinary assessments to identify patients' mental health needs and develop tailored care plans.
- Collaborate with other healthcare professionals, including psychiatrists, psychologists, and social workers, to ensure seamless care coordination.
- Stay up-to-date with the latest mental health research, best practices, and service developments to continuously improve patient care.
The successful candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with patients, colleagues, and stakeholders. You will also be committed to providing high-quality patient care, working collaboratively as part of a dynamic team, and adhering to professional standards and guidelines.
Requirements- Current registration with the Nursing and Midwifery Council (NMC).
- Proven experience in a community mental health setting, with a focus on recovery-oriented practice.
- Strong understanding of adult mental health principles, including assessment, diagnosis, and treatment planning.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
