Job Overview

The Care Coordinator Lead plays a pivotal role in our Discharge to Assess (D2A) pathways, working closely with hospital and community-based partners. This position is responsible for overseeing a team that delivers high-quality reablement support to individuals in their own homes.

Key Responsibilities

Requirements

  • Strong leadership and change management skills.
  • Familiarity with the NHS and local authority systems.
