End of Life Care Coordinator

In this role, you will be responsible for providing holistic nursing care to patients at the end of life. This includes offering symptom control, supporting patients and their families, and responding to urgent contacts.
As a member of our team, you will work collaboratively with colleagues to ensure seamless care delivery. Your strong communication and interpersonal skills will enable you to build trusting relationships with patients, families, and healthcare professionals.
This role requires a high level of emotional intelligence, empathy, and compassion. You will be expected to manage complex situations and make sound judgments in a fast-paced environment.
The ideal candidate will possess up-to-date knowledge of palliative care principles and practices. A degree in palliative/cancer care or equivalent is desirable.
