Leaving Care Personal Advisor Role

Service Care Solutions - Social Work is seeking a skilled and compassionate Leaving Care Personal Advisor to support care leavers in their transition to adulthood.

Main Responsibilities:

  • Contribute to the development and implementation of Pathway Plans through direct interventions and engagement with care leavers.
  • Provide guidance and support to care leavers in developing practical independent living skills, managing tenancies, and financial capabilities.
  • Offer coaching and motivational support to help care leavers build confidence and self-esteem, enabling them to succeed in adulthood.
  • Safeguard and protect care leavers as they transition to independence, ensuring they are effectively engaged, in suitable accommodation, and accessing education, training, or employment appropriate to their abilities and aspirations.
  • Collaborate with colleagues across agencies to coordinate the provision of relevant services and ensure care leavers make use of them.

Requirements:

  • Enhanced DBS
  • Experience in a similar role
  • Educated to A level (or equivalent) or extensive experience of working directly with children, young people, their parents, and carers to effect positive change
