Community Nursing Opportunity

We are seeking a skilled Community Nurse to join our Rapid Response team, providing nursing interventions to patients in their own homes. As a member of our team, you will work collaboratively with health and care providers to deliver high-quality care, ensuring that patients receive the best possible support to maintain their independence.

The ideal candidate will have a passion for assessing, planning, and providing evidence-based nursing care for patients with a wide range of conditions. You will work efficiently and creatively with multi-professional colleagues to help patients avoid hospital admissions and facilitate timely and well-supported discharge back home.

Main Responsibilities
  • Assess and manage patients with long-term conditions, palliative care needs, and wound care.
  • Provide IV antibiotics, line care, wound care, IM/SC injections, Peg Feeds, Venepuncture, Cannulation, and Palliative care.
  • Work collaboratively with the multidisciplinary team to deliver healthcare services.
  • Support patients, families, and carers in dealing with people with nursing needs.

Person Specification
  • Registered Nurse with a degree in nursing or equivalent.
  • Registration with the NMC.
  • Working towards Mentorship qualification.
