Job Overview

We are seeking a skilled Advanced Community Practitioner to join our dynamic GP surgery teams across locations in Nottingham City. The successful candidate will provide high-quality care to patients across a range of conditions, working autonomously and collaboratively with GPs and the wider practice team.

About the Role

This full-time position is based at Grange Farm Medical Centre, which serves a deprived area of Nottingham. The practice has a history of achieving exemplary patient care (outstanding CQC rating) and provides the lead care home role for City South PCN.

The ideal candidate will have extensive clinical experience in primary care, urgent care, or emergency medicine, as well as a Master's degree in Advanced Practice or equivalent experience.

A competitive salary of £62,000 - £84,000 per annum reflects the importance of this role in providing resilient care for the city.

Key Responsibilities:

  • Prescribe medication in accordance with national and local guidelines.
  • Provide health promotion, disease prevention, and patient education.
