Pharmacist - Medication Optimisation Specialist

The NHS is seeking a dedicated and highly skilled Pharmacist - Medication Optimisation Specialist to join our esteemed clinical pharmacy team in Buckinghamshire. As a key member of the Medicine for Older People Directorate, you will play a pivotal role in delivering exceptional pharmaceutical care services.
Main Responsibilities- Specialist Pharmaceutical Care Service Delivery: Work closely with multidisciplinary teams to provide tailored medication optimisation services, ensuring patients receive the best possible care.
- Patient Identification and Prioritisation: Proactively identify high-risk patients requiring specialist input and deliver prompt, effective pharmaceutical advice.
- Medicines Management Duties: Perform medicines reconciliation, optimisation, and improvement activities to enhance prescribing practices and facilitate timely discharges.
- Clinical Information Provision: Deliver accurate, comprehensive information and training to medical and nursing staff, as well as patients, regarding medications, therapeutic effects, monitoring, and potential adverse effects.

Requirements- Education and Qualifications: Registered Pharmacist with membership of the General Pharmaceutical Council.
- Experience: Strong background in hospital pharmacy, preferably within the Medicine for Older People specialty.
- Skills and Knowledge: Excellent communication, leadership, and prioritisation skills; ability to work effectively within a team and adapt to changing situations.
