As an Extended Personal Advisor, you will play a vital role in building meaningful relationships with care leavers, providing targeted support and guidance to help them achieve their goals and reach their full potential.

Key Responsibilities

  • Provide high-quality support and guidance to care leavers, helping them to develop the skills and confidence they need to succeed in adulthood.
  • Work closely with a range of professionals, including social workers, teachers, and healthcare professionals, to ensure that care leavers receive the best possible support.
  • Develop and implement personalized support plans, taking into account the unique needs and circumstances of each care leaver.
  • Monitor and evaluate the effectiveness of support plans, making adjustments as needed to ensure that care leavers are achieving their goals.
  • Collaborate with other teams and agencies to ensure that care leavers receive a seamless and coordinated service.

Requirements

  • Relevant experience of working with care leavers, either in a paid or unpaid capacity.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with care leavers, colleagues, and other professionals.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach to work.
  • Knowledge of legislation, policy, and guidance related to care leavers, with the ability to apply this knowledge in a practical and effective way.
  • Valid driving license and access to a vehicle, with the ability to travel to various locations as needed.
