Job Summary

We are seeking a skilled and compassionate Band 5/6 Health and Wellbeing Nurse to join our team at Northern Lincolnshire and Goole NHS Foundation Trust. Key Responsibilities
  1. Provide expert nursing care to patients with complex health needs, working collaboratively with multidisciplinary teams to develop and implement individualized care plans.
  2. Contribute to the development and delivery of health and wellbeing programs, promoting healthy lifestyles and disease prevention among patients and the wider community.
  3. Work closely with patients, families, and carers to identify and address health and wellbeing needs, providing education and support to empower individuals to take control of their health.
  4. Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care and safety.
  5. Collaborate with colleagues to develop and maintain a culture of excellence, respect, and inclusivity, promoting a positive and supportive work environment.

Requirements
