Job Overview:

Service Care Solutions is seeking an experienced Family Support Practitioner to join our team. As a Family Support Practitioner, you will work with vulnerable families to provide relationship-focused social work support and develop effective strategies to address risk factors impacting their well-being.

Key Responsibilities:
  • To conduct home visits and assessments to identify family needs and develop comprehensive support plans
  • To collaborate with other professionals to ensure effective case management and risk reduction
  • To maintain accurate records of all case-related activities and contribute to regular progress reports
Requirements:
  • Degree in Social Work and current registration with Social Work England
  • Enhanced DBS disclosure and access to a reliable vehicle for travel between locations
  • Proven experience in relationship-focused social work, with a strong emphasis on case management and risk reduction
What We Offer:

  • Payroll service twice a week and annual training budget of up to £250
Estimated Salary:

£34,000 - £40,000 per annum
