Physiotherapist Advanced Practitioner Opportunity

We are seeking a highly skilled and motivated Physiotherapist Advanced Practitioner to join our multidisciplinary team at Wessex LMCs Ltd.

Key Responsibilities
  • Work independently to assess, diagnose, and manage patients, prioritizing and managing a caseload of registered patients.
  • Receive patients who self-refer or from clinical professionals, referring to other health professionals as needed.
  • Develop integrated care programmes in partnership with patients, providing first-line treatment options, including self-management and social prescribing.
  • Utilize full scope of practice, developing skills in independent prescribing, injection therapy, and investigation to make professional judgments and decisions.
  • Manage complex interactions, working with patients with psychosocial and mental health needs, referring onwards as required.
  • Communicate effectively with patients and their carers, providing complex and sensitive information regarding diagnoses, pathology, prognosis, and treatment choices.
  • Implement effective clinical governance, including regular audit and evaluation, supervision, and training.
  • Agree with patients on appropriate support for self-management through referral to rehabilitation-focused services and social prescribing.
  • Design and implement plans to facilitate behavioral change, optimize patient physical activity and mobility, and reduce the need for pharmacological interventions.
  • Be accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking and engagement in evidence-based practice.
  • Provide leadership and support on MSK clinical and service development across the PCN, alongside learning opportunities for the multidisciplinary team.
  • Develop relationships and a collaborative working approach across the PCN, supporting the integration of pathways in primary care.
  • Liaise with secondary and community care services, using local social and community interventions to support patient management.
