The Role:

We are seeking a highly motivated and compassionate Adult Care Coordinator to join our team. This role offers an exciting opportunity to work with vulnerable adults, promoting their independence and well-being.

Key Responsibilities:

  • Develop and implement individualized care plans, taking into account the unique needs and goals of each service user
  • Build strong relationships with service users, families, and other stakeholders, ensuring seamless communication and collaboration
  • Provide emotional support and guidance to service users, empowering them to make informed decisions about their care
  • Monitor and report any changes in service user circumstances, ensuring timely interventions and adjustments

Requirements:

  • NVQ in Health and Social Care or willingness to work towards one
  • Experience in supporting adults with challenging behavior, learning disabilities, and/or mental health issues
  • Calm under pressure, confidence to work alone, and excellent communication skills
  • Passion for working with vulnerable adults and commitment to safeguarding
