Job Description

This is a unique opportunity to work as a Senior Nurse in our Short Term Assessment, Rehabilitation and Reablement Service (STARRS). You will be responsible for providing high-quality nursing care to patients within their own homes, working closely with other healthcare professionals to achieve optimal patient outcomes. This role requires you to have a broad range of experience and excellent communication skills, with a strong focus on leadership and management.

Main Responsibilities
  • Provide evidence-based nursing care to patients in their own homes, following agreed care plans and protocols.
  • Utilise digital technology to monitor patients remotely, using approved health applications to track clinical parameters and take appropriate action when necessary.
  • Work collaboratively with other virtual ward specialties services, ensuring that all clinical data is captured and patients are monitored accordingly.
  • Undertake comprehensive assessments, planning, implementation, evaluation, and auditing of clinical care provision in primary, secondary, and tertiary care settings as required.
  • Optimise hospital bed utilisation by ensuring timely and safe transfers or discharges of patients to the most appropriate environment, preventing unnecessary hospital admissions where possible.

  • Broad range of experience and excellent communication skills
  • Registered car driver with clean UK licence
Salary and Benefits

The estimated salary for this position is around £40,000-£60,000 per annum, depending on experience. We offer a range of benefits, including a lease car facility, to support your career development and well-being.
