Job Title: Young Person Support Worker (Primary Age)

Join our team at Freedom Care Group as a Young Person Support Worker (Primary Age) and make a meaningful difference in the life of a young person with additional needs and challenging behaviors.

About the Role:

We are seeking a highly motivated and compassionate individual to provide support and guidance to a young person with primary school age, who is neurodiverse and displays challenging behaviors. As a Young Person Support Worker, you will be responsible for creating stability and developing life skills and opportunities through community and home-based activities.

Key Responsibilities:
  • Provide one-to-one support and guidance to the young person
  • Develop and implement personalized plans to meet the young person's needs
  • Engage in community and home-based activities to promote social skills and independence
  • Collaborate with the case manager and out-of-hours team to ensure seamless support
  • Participate in training and professional development to enhance skills and knowledge
Requirements:
  • Experience working with young people with additional needs and challenging behaviors
  • Level 3 in health and social care or equivalent qualification
  • DBS on the update system (or willingness to apply)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
