**Job Description**

We are seeking a Support Services Coordinator to join our team in Motherwell, Scotland. As a Support Services Coordinator, you will play a key role in ensuring the smooth delivery of our community-based support services, working closely with clients, families, and external agencies to promote independence and well-being.

Key Responsibilities:

  • Coordinate the delivery of individualized support plans, working closely with clients and their families
  • Develop and implement strategies to promote independence and well-being
  • Collaborate with external agencies, such as healthcare professionals and social workers
  • Document client progress and participate in regular review meetings
  • Contribute to the development and implementation of new initiatives and projects

Requirements:

  • Experience working in social care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work varied shifts, including evenings and weekends
  • Demonstrated understanding of the importance of confidentiality and discretion

Salary and Benefits:

  • A competitive salary of £22,000 - £26,000 per annum (dependent on experience)
