Job Summary: We are seeking a skilled Registered Nurse to join our team as a Caregiving Professional. In this role, you will be responsible for coordinating patient care in the home setting, conducting comprehensive health assessments, and providing oversight to non-skilled staff members.

Responsibilities:
  • Conduct thorough health and psychosocial assessments of patients in their homes
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Provide education and support to patients and their families on disease management and self-care techniques
  • Coordinate with other healthcare professionals to ensure seamless care transitions
  • Supervise and train non-skilled staff members to provide high-quality patient care

Requirements:
  • Graduate from an accredited School of Nursing with a current RN license in the state of Pennsylvania
  • At least one year of experience in a community/home health agency or home setting preferred
  • Knowledge of Medicare and Medicaid guidelines
  • Detail-oriented with excellent communication and interpersonal skills
  • CPR certification and valid driver's license required
