About the Role

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at Leaders In Care Recruitment Ltd. As a Community Nurse Case Manager, you will play a vital role in supporting clients in the North West to live independently at home and within their communities.

Key Responsibilities:
  • Manage a team of experienced Support Workers to deliver high-quality care and support services
  • Conduct comprehensive assessments to identify clients' needs and develop personalized care plans
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless care coordination
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed
  • Provide leadership and guidance to the care team to ensure excellent patient outcomes
Requirements:
  • Registered General Nurse (RGN) or Registered Sick Children's Nurse (RSCN) qualification
  • Proven experience in complex care and case management
  • Strong leadership and team management skills
  • Excellent communication and interpersonal skills
