Job Summary:
We are seeking a highly motivated and dedicated Care Worker to join our team at Service Care Solutions. As a Care Worker, you will play a vital role in providing a comprehensive accommodation-based support service to vulnerable adults and young people living in our singles hostels.
Main Responsibilities:
- Needs Assessment and Support Planning: Conduct regular assessments and reviews of needs on a casework basis, conduct key work sessions, and develop support plans to actively identify and enable service users to secure appropriate support from partners, including physical and mental health services, drug and alcohol services, and employment, training, and resettlement services.
- Accommodation-Based Support: Provide a high-quality and responsive accommodation-based support service to assist service users in developing the personal and social skills necessary to maintain their accommodation, improve their health and wellbeing, improve their employability, and financial inclusion, and decrease any negative impact of their behavior on their locality.
- Risk Management: Carry out regular and comprehensive assessments of risk associated with service users, including Safeguarding, and ensure that identified risks are actioned and recorded in accordance with service policy and procedure and Commissioning requirements.
Requirements:
- Enhanced DBS: A valid Enhanced DBS check is required for this role.
- Experience: Previous experience in a similar role is essential.
