Home Health Care Specialist

We are seeking a highly skilled Physical Therapist to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide and oversee home health services for clients within the community.
Job DescriptionThe successful candidate will have at least two (2) years' experience in a healthcare facility or home care field. They will be responsible for performing initial and periodic assessments of clients to determine their care needs, implementing interventions, and evaluating the effectiveness of care to achieve positive outcomes.
Additionally, they will collaborate with physicians/non-physician practitioners and the home health team to develop and update the client's home health plan of care at least every 60 days or more often as changes occur.
Our ideal candidate will possess critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgments to provide safe and effective client care.
