Job Description

Role Overview

The Intensive Enablement Service is a dynamic and supportive team that focuses on empowering individuals with diverse needs, including Learning Disabilities, Autism, and Mental Health. Our goal is to provide personalized support, enabling individuals to build on their independent living skills and problem-solving abilities.

We work closely with individuals, their families, and other stakeholders to develop tailored support plans, addressing their unique needs and goals. Our team is committed to fostering a culture of inclusivity, respect, and empowerment, ensuring that every individual feels valued and supported.

Key Responsibilities

  • Provide intensive support to individuals with complex needs, focusing on building their daily living skills and promoting positive wellbeing.
  • Develop and implement personalized support plans, working collaboratively with individuals, their families, and other stakeholders.
  • Conduct regular assessments and reviews to ensure the effectiveness of support plans and make adjustments as needed.
  • Collaborate with other teams and services to ensure a seamless and coordinated approach to support.
  • Participate in regular training and professional development opportunities to enhance skills and knowledge.

Requirements

  • A full driving license and access to a vehicle for work-related purposes.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and stakeholders.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the needs of individuals and the service.
  • Knowledge of Learning Disabilities, Autism, and Mental Health, with a commitment to person-centered and strengths-based practice.
