Palliative Care Team Member

We are looking for a skilled Palliative Care Team Member to join our multidisciplinary team at Coordinated Care Services, Inc. This role plays a vital part in delivering comprehensive mental health services to patients requiring palliative care.
Palliative Care Team Member ResponsibilitiesThe successful candidate will be responsible for:
- Screening, assessing, and developing individualized treatment plans for patients with complex mental health needs
- Collaborating with physicians, nurses, and other healthcare professionals to provide integrated care
- Conducting regular case reviews and engaging in family meetings to ensure patient-centered care
- Maintaining accurate documentation of interventions and patient progress
To succeed in this role, you will require a Master's degree in social work from an accredited institution, relevant experience working with critically ill patients, and licensure as a social worker. A strong commitment to delivering empathetic and supportive care is essential.
Our competitive annual salary for this position is $72,419.
