About the Role

We are seeking a highly skilled and dedicated Qualified Practitioner to join our Learning Disability team at Lincolnshire County Council. As a Qualified Practitioner, you will play a vital role in supporting individuals with learning disabilities and their families, ensuring that they receive the best possible care and support.

Main Duties:
  • Manage an allocated caseload of clients from across the Learning Disability service, providing support and guidance to individuals and their families.
  • Review and assess the current care and support plans in place, identifying areas for improvement and developing strategies to enhance the quality of care.
  • Collaborate with key stakeholders, including healthcare professionals, social workers, and families, to ensure that the care and support plans are tailored to meet the individual's needs.
  • Complete DoLS applications, gathering and analyzing evidence to support the application process.
  • Maintain accurate and up-to-date records, including case notes and reports, to ensure that the individual's care and support plans are effectively documented.
  • Communicate effectively with individuals and their families, providing clear and concise information about the care and support plans and any changes that may be made.
About You

We are looking for a qualified and experienced practitioner who is passionate about delivering high-quality care and support to individuals with learning disabilities. You will need to hold a Social Work or Nursing qualification and be registered with the relevant professional body.

Suitable candidates will possess excellent communication and interpersonal skills, with the ability to work effectively with individuals and their families. You will also need to be able to work independently, prioritizing tasks and managing your workload effectively.
