Advanced Clinical Practitioner Opportunity

We are seeking an experienced Advanced Clinical Practitioner to join our team at Primary Care 24. As a key member of our Out of Hours Services, you will play a vital role in delivering high-quality patient care and contributing to the growth and development of our social enterprise.

About the Role

As an Advanced Clinical Practitioner, you will be responsible for assessing, diagnosing, and providing advice, care, and management to patients presenting with undiagnosed or undifferentiated illness via telephone consultations or face-to-face assessments. You will work autonomously, supported by GP colleagues in the out-of-hours setting, and demonstrate clinical reasoning, critical thinking, and leadership skills.

Key Responsibilities
  • Undertake face-to-face and telephone triage consultations
  • Interpret pathology/radiology results/reports
  • Prescribe and review medication
  • Plan, implement, and evaluate an effective management plan for the patient

Location and Shift Times
