Job Summary

We are seeking an experienced and dynamic Primary Care Nurse Manager to join our team at Carlton Group Practice. As a key member of our clinical leadership team, you will be responsible for providing managerial and professional leadership to our nursing and nursing support team.

Main Responsibilities
  • Provide managerial and professional leadership to the nursing and nursing support team
  • Work closely with the Business and Operational Managers to deliver, plan and future-proof services
  • Lead the team to provide an exemplary clinical service while ensuring practice targets are maintained and achieved to optimal levels
  • Accountable for the delivery and standards to ensure safe, effective, person-centered care
  • Manage and review performance and absence management
  • Act as the Infection Prevention Control (IPC) Lead and work to the IPC Policy
  • Develop and implement operational plans and strategies for service delivery
  • Develop, implement, and embed health promotion and wellbeing programmes
  • Implement and evaluate individual treatment plans for chronic disease patients
  • Identify, manage, and support patients at risk of developing long-term conditions
  • Provide routine nursing care to patients as required
  • Diagnose and manage acute and chronic conditions
  • Prescribe medication for patients in accordance with PGDs and own scope of practice
  • Review patient medications to enhance compliance
  • Oversee the provision of wound care services to patients
  • Co-ordinate the provision of travel medicine services
  • Process pathology results as required
  • Oversee the provision of chronic disease clinics
  • Maintain accurate clinical records
  • Ensure read codes and templates are used effectively
  • Develop, implement, embed, and participate in well-person clinics
  • Chaperone patients where necessary
  • Assist GPs with minor surgery and contraception when required
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess, and refer patients presenting with mental health needs
  • Implement vaccination and screening programmes for adults and children
  • Liaise with external services/agencies to ensure patient support
  • Delegate clinical responsibilities appropriately
  • Support the clinical team with safeguarding matters
  • Understand practice and local policies for substance abuse and addictive behaviour
  • Deliver opportunistic health promotion
  • Review clinical practices and respond to national policies and initiatives
  • Co-ordinate the nursing team rota
  • Facilitate placement of Student Nurses within the Practice
  • Develop and review audit protocols and process
  • Develop, review, and implement nursing protocols
  • Participate in local initiatives to enhance service delivery and patient care
  • Support and participate in shared learning within the practice
  • Participate in the review of significant and near-miss events
  • Lead the planning and implementation of changes within the nursing team
  • Support the practice team with reviewing and implementing practice policies and protocols
Person Specification
  • Experience of working within a primary care environment
  • Experience in all areas of Chronic Disease Management, including Diabetes
  • Experience of working autonomously
  • Leadership experience
  • Experience of Infection and Prevention Control Management
  • Desirable: Experience of leading and managing a team in a primary care environment
  • Desirable: Experience of working as a Practice Nurse or Community Nurse
Requirements
  • Registered Nurse
  • Educated to a good standard
  • Evidence of Continued Professional Development
  • Desirable: Post Graduate Diploma or degree (Chronic Disease Management)
  • Desirable: Nurse Prescriber
