About the Role
Southport and Formby Health (GP Federation) is seeking a highly skilled Learning Disability Nurse to join our Enhanced Health in Care Homes Team. As a key member of the team, you will play a vital role in promoting the health and wellbeing of individuals with learning disabilities living in care homes.
Key Responsibilities
- Work collaboratively with care home staff to develop and implement individual care plans that meet the unique needs of residents.
- Provide clinical support and guidance to care home teams to ensure high-quality care provision.
- Conduct holistic assessments and make independent clinical decisions to promote the health and wellbeing of residents.
- Work closely with the Primary Care Network (PCN) to identify gaps in local service provision and develop strategies to reduce health inequalities.
- Develop and deliver education sessions to care home staff and other stakeholders to promote health education and screening processes.
Requirements
- Current qualification and registration with the NMC.
- Excellent interpersonal and communication skills.
- Strong IT skills and experience of recording care plans electronically.
- Knowledge of up-to-date learning disability policy and EMIS clinical software.
