About the Role

Practice Plus Group is looking for a dedicated Primary Care Nurse to join our team at HMP Brixton, a secure environment in London. As a Release and Transfer Nurse, you will be responsible for managing the assessment of health needs, planning, delivery, and evaluation of care for the prison population.

You will provide care and planning for adult male prisoners from reception until release or transfer to another establishment, working collaboratively with patients and other agencies to support a safe transition back into the community or other establishment.

We offer a competitive salary of up to £53,172 per annum, depending on experience, as well as a range of benefits, including bespoke induction, competency framework, regional and national career development opportunities, and a Learning Management System to address your learning needs.
