About the Role
We are seeking an experienced Clinical Nurse Specialist to join our Blood and Bone Marrow Transplant team in Cardiff and Vale University Health Board.
As a Clinical Nurse Specialist, you will be responsible for providing high-quality, patient-centered care to individuals with Acute/Chronic Graft Versus Host Disease (GVHD).
You will work closely with the multidisciplinary team to coordinate day unit and clinic-based post-transplant care, completing clinical skills tasks such as spirometry and providing support to patients and their families throughout their treatment journey.
Main Responsibilities
- Deliver high-quality and efficient haematology day centre/GVHD clinic and inpatient care for GVHD patients and their families.
- Assess, plan, deliver, and evaluate patient care using a holistic and individualized approach based on best practice and clinical effectiveness.
- Liaise closely with the AHP team and coordinate care as recommended.
- Provide continued support to patients throughout their GVHD treatment journey, ensuring timely and efficient care delivery.
- Work with the ANP/Senior/Lead nurse and BMT Consultant lead to develop and update documentation against agreed criteria, maintaining a governance framework around practice.
- Provide patients with information, education, and psychological support throughout the course of their disease and treatment.
- The post holder will be required to work clinically in the outpatient setting.
- The successful candidate will be required to enhance their clinical role by training in practical procedures such as cannulation/venepuncture/chemotherapy/care of central lines and factor concentrate administration.
- The applicant would be expected to cross-cover other team CNSs and take part in the nurse-led late effects clinic.
- Maintain up-to-date nursing records and care plans.
