Job Title: Clinical Nurse Specialist

Job Summary:

Gray Healthcare is seeking a skilled Clinical Nurse Specialist to lead on clinical assessments and deliver individualized support packages. As a key member of our team, you will work closely with the Assessment and Contracts Manager to ensure packages are deliverable and in line with our model of support.

Main Responsibilities:

  • Completing desk-based and face-to-face assessments of referred individuals for support.
  • Ensuring an APIE approach - Assessing, planning, implementing, and evaluating.
  • Reviewing reports and evidence outcome measures to inform clinical practice, business performance, and quality measures.
  • Able to identify and mitigate complex risks for a community setting.
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
  • Attend internal and external MDT meetings to further inform assessment and risk mitigation.
  • Building professional working relationships.
  • Effective time management.
  • Record keeping, ensuring accurate, safe, and contemporaneous records are maintained.

Requirements:

  • Registered Mental Health Nurse or Occupational Therapist.
  • Experience of assessing and working with individuals with complex needs, including mental health, learning disability, ABI, personality disorder, autism, and forensic backgrounds.
  • Understanding of continuing healthcare or transition of individuals moving from hospital/residential settings into the community.
  • Applied knowledge of the Mental Health Act, CTOs, and the Mental Capacity Act, Deprivation of Liberty Safeguards, and Best Interest Decision making.
  • Expertise in assessment and management of risk, with strong report writing skills and computer literacy.
  • National travel and overnight hotel stays may be required.
  • Full UK Driving Licence and access to a vehicle.
